Ichigo blames himself for his mother's death - he saw a ghost about to fall into the water by a railway track (he couldn't tell the difference) and rushed to help her, but his mother got hit by a train while trying to stop him. While Rukia keeps watch on Ichigo, she encounters an upperclassman from Shinigami training, Eichirou, who has come to take her back to Soul Society - Ichigo foils his attempts, but has another problem at hand when a Hollow goes after Yuzu and Karin at his mother's grave.
Eichirou's a bit of a funny bod - switching suddenly from complete slapstick to deadly (if you'll excuse the pun) seriousness makes him a poorer character in my opinion. In other places in Bleach this combination works, but it just seemed a little out of place for the moment to be honest. The fact that the preview was played totally straight this time is definitely quite telling about where this arc is going, and it should be interesting to see how it plays out.
